Physical Therapist

MAIN FUNCTION: Provides prescribed treatments of physical therapy, which involve a complex and sophisticated level of modalities, designed to restore the patient to his/her maximum rehabilitation potential. ESSENTIAL TASKS: Performs direct physical therapy services as planned by the Agency staff and certified by the patient’s physician. Assess, reports and records patient’s status and progress to the physician and the health care team. Teaches and supervises Nurses, Home Health Aides, family members and patients to provide for a continuous physical therapy program. Participates in clinical conferences and in service programs to share and stimulate the health care team in meeting rehabilitation goals. Delivers services to patients in accordance with accepted standards of treatment specifically effective for the condition from which the patient suffers. Assists with the development and writing of the plan of treatment in collaboration with the health care team and physician. Completes appropriate clinical records, including the one-page visit note, documenting patient limitations and progress, and submits to the Alternate Administrator or Clinical Care Manager/Supervisory RN within 24 hours of the visit. Is a member of the Clinical Review Committee and attends meetings as designated by the Administrator, Alternate Administrator, and/or Clinical Care Manager/Supervisory RN. Develops and instructs Agency staff and family members in maintenance therapy programs as approved by the physician. Does evaluations of patients only at the direction of the Agency via written referral. Presents Inservice education to rehabilitation and nursing staff. Assumes responsibility for equipment signed out for patient use. Any other duties as assigned by the Administrator, Regional Clinical Director, or Clinical Director. Complies with departmental and Company policies and procedures. Full-time Registered Physical Therapist must make a minimum of 35 visits per week. EDUCATION AND EXPERIENCE: Graduate of an approved Physical Therapy Program. Licensed as a Registered Physical Therapist in the State. Current Certification in CPR. One year of physical therapy experience. Must show evidence of liability insurance coverage, if contractual. SKILLS AND ABILITIES: Is able to operate in various environments. Is able to stand 3 or 4 hours at a time. Is able to push and pull 100 pounds. Must be flexible to crouch and crawl around patients. Has good manual dexterity and feeling for massaging and rubbing. Has good eye and hand coordination. Can communicate effectively with patients, co-workers, and the general public. Can analyze situations well. Can handle multi-tasks. Is able to operate a vehicle. Has excellent guest relation skills. Can manage time effectively. Can follow Universal Precautions. Is able to work as scheduled and report to work on time.
