Express Care and Patient Access Center Opens in Nacogdoches
Whether you’re coming down with a sudden illness or have a serious injury, a new walk-in health care office in Nacogdoches offers convenient, affordable health care in collaboration with a network of area physicians.
Now open in the University Park Plaza Shopping Center, 3205 N. University Dr., Ste. E, STRiVE Express Care is open late seven days a week and provides after-hours and urgent care in the comfort of a centrally located office. The clinic is designed to reduce costs while improving patient outcomes by working with primary and specialty care physicians to provide efficient, coordinated health care.
In addition to the convenient hours and location, STRiVE Express Care features in-house labs, X-rays and EKG monitoring. STRiVE Express Care accepts cash payments and insurance and Medicare, and is much less expensive than a visit to the hospital ER or a traditional urgent care clinic.
While much of the focus is on making it easier for patients to get in the door, it’s what happens after the initial visit that sets STRiVE Express Care apart. STRiVE Express Care is able to make sure your physician knows about your visit and has a record of the treatment and medications you received. That follow-up is especially important for patients who were recently discharged from the hospital or have had surgery.
For patients who have just left the hospital, a comprehensive follow-up evaluation is one of the best ways to ensure compliance with the discharge plan, reduce readmission and improve patient health. STRiVE Express Care works closely with a team of nurse care managers, health coordinators, clinical pharmacists and local physicians to streamline the discharge transition from the hospital back into the hands of your physician.
For patients who develop urgent concerns after surgery, STRiVE Express Care works closely with the patient’s physician through communication and telehealth platforms to provide care and reassurance while coordinating the patient back to their physician.
STRiVE Express Care was founded as an initiative of the STRiVE Clinical Network, a physician-led, non-profit accountable care organization. STRiVE Clinical Network is made up of more than 121 primary care physicians throughout deep east Texas and is governed through a Board of Directors and subcommittee structure, comprised entirely of physician members.
